Game Over is a standard feature in most video games that tells the player that the game has ended, due to a number of factors.

While a Game Over can be obtained for any number of reasons, they are generally negative in connotation. This generally comes in one of two forms: either the player has lost all their lives/health, or the player has failed to complete a crucial objective. However, this term can also be used to indicate completion of the game.
Generally, when the player obtains a Game Over, they will restart from the last save point.

Permadeath is a variation of Game Over which forces the player to start from the beginning if they choose to resume gameplay.
Permadeath is a defining trait of "Hardcore Mode", a difficulty level present in some video games, where it is accompanied with high difficulty levels.
